What Is Your TCM Body Constitution? A Practitioner's Guide to the Five Elements

In Traditional Chinese Medicine, health is not one-size-fits-all.

Your constitution 体质 (tǐ zhí) is the way your body is naturally built and how it responds to the world around it. It shapes everything: your energy levels, your skin, your sleep, how easily you catch colds, how you process stress. Understanding it is the first step in knowing what your body actually needs.

The Five Elements framework (五行, wǔ xíng) is one of the most enduring tools in Chinese medicine. Each element — Wood, Fire, Earth, Gold (Metal), and Water — corresponds to an organ system, a set of functions, a season, and a pattern of strengths and vulnerabilities. Your dominant element is not a limitation. It is a map.

木 Wood — Liver & Gallbladder

Your nature: Driven, decisive, creative. Wood types are natural initiators — they move toward goals with clarity and force. When in balance, they are visionary and flexible. When out of balance, they become irritable, tense, and prone to frustration.

Signs of imbalance: Frequent headaches (especially at the temples), eye fatigue, tight shoulders and neck, disturbed sleep around 1–3am, PMS or irregular cycles, a tendency to feel emotionally wound up.

What helps: Herbs that support liver qi movement and blood nourishment — wolfberries (goji), red dates, chrysanthemum, cassia seeds. Light, regular meals. Movement. Rest before midnight.

火 Fire — Heart & Small Intestine

Your nature: Warm, intuitive, expressive. Fire types connect easily with others and feel deeply. At their best, they are charismatic and joyful. When depleted, they become anxious, scattered, and prone to palpitations.

Signs of imbalance: Difficulty falling asleep or staying asleep, heart palpitations, a restless or easily over-stimulated mind, excessive dreaming, feeling “wired but tired.”

What helps: Herbs that calm the heart and nourish shen (spirit) — lotus seeds, lily bulb, longan, poria mushroom. A consistent sleep schedule. Quieter evenings.

土 Earth — Spleen & Stomach

Your nature: Grounded, nurturing, methodical. Earth types are the ones people lean on — steady, thoughtful, reliable. In balance, they are centred and caring. Out of balance, they overthink, feel heavy, and struggle with digestion.

Signs of imbalance: Bloating after meals, loose stools or sluggish digestion, fatigue after eating, sugar cravings, a tendency to worry or ruminate, feeling heavy or damp.

What helps: Herbs that strengthen spleen qi and resolve dampness — coix seed (yi yi ren), Chinese yam (shan yao), white atractylodes (bai zhu), dried tangerine peel (chen pi). Warm cooked foods. Eating without distraction.

金 Gold (Metal) — Lung & Large Intestine

Your nature: Precise, principled, refined. Gold types have high standards — for themselves and for the world around them. In balance, they are clear-headed and resilient. Out of balance, they are prone to grief, respiratory issues, and difficulty letting go.

Signs of imbalance: Frequent colds or respiratory infections, dry skin and throat, constipation, a tendency to hold onto sadness or regret, low immunity across seasons.

What helps: Herbs that tonify lung qi and nourish yin — fritillary bulb (chuan bei mu), lily bulb, snow fungus, apricot kernel, pear. Breathwork. Seasonal care in autumn.

水 Water — Kidney & Bladder

Your nature: Deep, perceptive, enduring. Water types carry a quiet strength and a long view. At their best, they are wise and calm under pressure. When depleted, they feel exhausted at the bone level — a tiredness that sleep does not fix.

Signs of imbalance: Lower back ache, knee weakness, frequent urination (especially at night), premature greying, low libido, fear and anxiety that feels foundational rather than situational, poor short-term memory.

What helps: Herbs that nourish kidney essence (jing) — black sesame, eucommia bark (du zhong), he shou wu (processed), cordyceps, black beans. Rest. Less cold food and drink. Consistent sleep hours.

A note from our practitioners

Most people carry a primary constitution with secondary influences — you may recognise yourself in more than one element. That is entirely normal. The goal is not a perfect match but a useful direction: which pattern resonates most, which signs feel most familiar.

Constitution is also not fixed forever. Stress, seasons, age, and life events all shift the balance. What your body needs this winter may be different from what it needed last summer. This is why in TCM, we tend to read constitution as a conversation, not a diagnosis.

If you would like a more personalised reading, our practitioners at Virtue TCM are available for consultation at our clinic in Mont Kiara, Kuala Lumpur.
